Personen, bei denen aufgrund der Art und Schwere ihrer Beeinträchtigungen eine ambulante oder teilstationäre Betreuung nicht mehr ausreichend ist, können vollstationär in Einrichtungen der Eingliederungshilfe betreut werden.
Bei der vollstationären Eingliederungshilfe wohnen Menschen mit einer körperlichen, geistigen, seelischen sowie mehrfachen Behinderung in einer stationären Einrichtung.

Aim of care
The aim of care work in long-term institutions is to offer people with impairments a place to live and to enable them to be as independent, self-determined and responsible as possible by means of housing, practical assistance and support, education and leisure activities, training and work.
Accommodation in a residential home
In principle, disabled people are accommodated in a residential home if they are working during the day in the workshops for disabled people or are taking part in another day-structuring measure. In some cases, the persons are also offered a day structure in the residential home.

Further procedure - individual help plan (IHP)
After you have submitted an application for integration assistance to the social services department, you will be notified in writing of an appointment for the assistance plan meeting.
The form "Individual Help Plan Jena (IHP 2017)" is used for the help planning (-> Downloads), which you fill out in preparation for the help plan meeting. In this form you will find a series of questions that are intended to record your need for help and your goals, wishes and needs, as well as the difficulties involved.
Your reference counsellor/social worker can support you in filling it out.
