Transcript of index card for driver's license

A file card transcript is an extract from the local driving license register of the driving license authority that issued the driving license and provides information about driving license data.

Requesting a copy of the index card from another authority

Your old (gray or pink) driving license was issued by another driving license authority. If so, please apply to the driving license authority in your original place of residence in writing or by telephone for a copy of the index card. Inform the employee that you now live in Jena and would like to apply for a new driving license.

Your old driving license was issued by the driving license authority in Jena.

Send your request to the city of Jena:

Necessary information

In order to process your request as quickly as possible and without additional queries, we require the following information - informally:

• Family name
• Former surnames (maiden name or married name)
• First name(s) (please state all)
• date of birth
• Place of birth
• Date of issue / list number of driver's license (if possible)
• Current home address
• Private telephone number for any queries

Please also provide us with the exact address and possibly the fax number of the driving license authority for which you require the file card copy. The copy of the index card will be sent directly to the driving license authority.

Residence in a member state of the European Union

If you have moved your place of residence to a member state of the European Union and require a copy of the index card to deal with your driving license matters there, please send us an informal letter and enclose a copy of your identity document.

The file card copy will be sent to your home address.

There is no charge for the issue of a transcript of a file card.
