Heilpraktiker - Application for a license
Anyone wishing to practise medicine without being licensed as a doctor requires a permit to do so. Corresponding applications must be submitted to the Health Department if you are resident in Jena or provide credible written assurance that you wish to establish yourself in Jena. The Health Department will decide on your application in consultation with the Erfurt health authority responsible for the knowledge test.

The examinations are held by the health authority in Erfurt in March and October of each year. You will receive the invitations directly from the Erfurt health authority.

Please use the form under Downloads to submit your application. The application must be accompanied by
- a certified birth certificate (can be obtained from the registry office in your place of birth)
- for married couples: the marriage certificate or an excerpt from the family register in certified form (to be obtained from the registry office of the place of marriage)
- a certified copy of the school leaving certificate
- a brief curriculum vitae in tabular form
- an official certificate of good conduct of document type "O", which is not older than 3 months at the time of application
- a medical certificate of medical fitness, which is not older than 3 months at the time of application (please use the form under Downloads)
- Proof of training courses for medical activities, if available
For the application for a sectoral license to practice medicine, restricted to the field of psychotherapy, please also enclose with your application
- a declaration that you wish to work exclusively in the field of psychotherapy
For the application for a sectoral license to practice medicine, restricted to the field of physiotherapy, please also enclose with your application
- a declaration that you wish to work exclusively in the field of physiotherapy
Applicants who are nationals of countries outside the European Economic Area (EEA) must also submit
- the residence permit
- the work permit
with the application.

Fees are charged on the basis of the Thuringian Administrative Costs Act (ThürVwKostG) in conjunction with the respective administrative cost regulations.
Please note that the fees will also be charged if your application is rejected or if the application is withdrawn before final processing.
