Among other things, the following family name changes are possible:
- Declaration of spouses on the use of the name
- Prefixing or appending the name of birth or the name used at the time of the determination of the married name
- Revocation of the aforementioned declarations
- Resumption of a previous name

The following list is not exhaustive. Please therefore enquire in advance about the documents to be submitted.
- Personal documents of the parties involved (identity card or passport)
- Marriage certificate
- possibly a legally binding divorce decree
- Possibly household certificate from the registration office
- Possibly a declaration of custody
